When to Use

  • Active suicidal ideation requiring immediate intervention
  • Acute psychotic episode requiring crisis stabilization
  • Severe panic or dissociative episode in session requiring extended crisis management
  • Urgent psychiatric emergency that is unscheduled

When Not to Use

  • Routine therapy sessions — even if the client discusses suicidal ideation, use 90834/90837 unless there is an acute crisis
  • Scheduled sessions that address crisis themes but are not emergent
  • Phone-based crisis intervention (separate crisis line codes exist)
  • When the crisis can be managed within normal session parameters

Common Denial Reasons

  • Documentation does not establish an acute psychiatric crisis (vs. a difficult session)
  • Billed for a routine scheduled session where the client was upset but not in crisis
  • Crisis nature not clearly differentiated from standard therapy
  • Billed alongside standard therapy codes (90834/90837) on the same day without clear separate encounters

Payer Notes

This is an emergency code — overuse will trigger audits. Documentation must clearly establish the crisis: imminent risk, acute psychiatric emergency, and why standard therapy was insufficient. Most payers cover without prior authorization given the emergency nature.
