How does the note generation work?

You type or dictate brief session observations — key topics, interventions, and client responses. WellNotes expands them into a fully structured clinical note matching your chosen template (SOAP, DAP, BIRP, etc.), using professional clinical language and proper formatting.

How does voice dictation work?

Click the microphone button and speak your session notes naturally. WellNotes transcribes your words and uses them to generate a complete clinical note. You can also upload a pre-recorded audio file. Voice dictation is available on Starter and Professional plans.

What note formats are supported?

We support 15+ formats: SOAP, DAP, BIRP, GIRP, SIRP, PIE, Treatment Plans, Intake Reports, Risk Assessments, and specialized templates for Couples, Family, Child-Family, Addiction, Group Therapy, and Supervision notes.

Do you offer a BAA (Business Associate Agreement)?

Yes. We provide a Business Associate Agreement to all subscribers. We take data security seriously. Contact support@wellnotesai.com to request your BAA.

Does my client data get sent to third parties?

Your session content is processed through a secure API to generate notes, but all content is sanitized of identifying information before it's sent. The AI provider does not use API inputs to train their models. No protected health information is stored on any external servers.

Does WellNotes integrate with my EHR?

WellNotes currently works as a standalone documentation tool. You can copy or export your completed notes directly into SimplePractice, TherapyNotes, or any EHR system. Direct integrations are on our roadmap.

What happens to my notes if I cancel?

Your notes remain accessible for 30 days after cancellation, giving you time to export everything. We also offer a one-click bulk export option so you can download all your notes before your account closes.

What is the difference between SOAP and DAP?

SOAP notes have four sections (Subjective, Objective, Assessment, Plan) while DAP notes combine the Subjective and Objective sections into a single "Data" section, resulting in three sections (Data, Assessment, Plan). SOAP provides a clearer distinction between what the client reports and what the clinician observes, making it stronger for insurance audits. DAP is faster to write and more flexible, which is why many private practice therapists prefer it. Both are widely accepted by insurance companies.

Which note format do insurance companies prefer?

Most private insurance companies accept SOAP, DAP, and BIRP notes. SOAP is the most universally recognized format across all healthcare disciplines, so it is the safest choice when dealing with multiple payers. Medicaid programs in many states prefer BIRP notes because they explicitly document the intervention used and the client's response. If you primarily work with self-pay clients, any format that meets your state licensing board requirements is acceptable.

What note format is best for ABA therapy?

SOAP notes are the most commonly used format in ABA therapy because the Objective section naturally accommodates behavioral data, frequency counts, and observable measurements that BCBAs and RBTs collect during sessions. The structured format also supports the data-driven nature of Applied Behavior Analysis. Some ABA practices also use BIRP notes, which work well because the Behavior and Response sections align with documenting target behaviors and client progress toward behavior reduction or skill acquisition goals.

What does BIRP stand for?

BIRP stands for Behavior, Intervention, Response, Plan. The Behavior section documents observable client behaviors and presenting issues. The Intervention section describes the specific therapeutic techniques the clinician used during the session. The Response section captures how the client responded to those interventions. The Plan section outlines next steps, including treatment plan updates and scheduling. BIRP is widely used in community mental health centers and Medicaid-funded programs because it clearly connects clinical interventions to treatment plan goals.
