Speech Evaluation CPT Codes: 92521, 92522, 92523 and 92524
Codes verified against ASHA's evaluation code guidance, its Medicare coding rules and 2026 fee schedule analysis, and the CMS speech-language pathology billing article A52866. Reviewed . See sources.
Which CPT code is used for a speech evaluation?
It depends on what was evaluated. 92521 is fluency, 92522 is speech sound production, 92523 is speech sound production together with language comprehension and expression, and 92524 is voice and resonance. 92522 and 92523 are not billed on the same day. 92526 is a swallowing treatment code, not part of this evaluation family.
Speech-language pathologists choose among four evaluation codes that were introduced on January 1, 2014 and replaced the older code 92506. Each code matches a type of evaluation rather than a length of time, so the right code comes from what the clinician actually evaluated and documented.
This guide covers each code with plain-language descriptions, the 92522 and 92523 rule, modifier 52, auditory processing evaluations, and where treatment code 92526 fits in. For treatment coding, see the CPT 92507 guide, and for swallowing evaluation see CPT 92610. Rules here come from ASHA and CMS sources linked below, and other payers set their own.
SLP evaluation codes 92521 to 92524 and related treatment code 92526 (paraphrased)
| Code | What it covers (paraphrased) | Key rule |
|---|---|---|
| 92521 | Evaluation of fluency, meaning how smoothly speech flows. | Untimed. CMS uses 92521 as its example of an untimed code reported with one unit. |
| 92522 | Evaluation of how speech sounds are produced. | Not billed on the same day as 92523. |
| 92523 | Speech sound production evaluation combined with evaluation of language comprehension and expression. | Includes the speech sound evaluation. Not billed on the same day as 92522. |
| 92524 | A behavior-based assessment of voice quality and resonance. | Does not include instrumental assessment. Untimed per ASHA. |
| 92526 | Treatment of swallowing problems or oral function for feeding. | A treatment code, not an evaluation. Swallowing evaluation is 92610. |
What is the difference between 92522 and 92523?
92522 reports an evaluation of speech sound production alone. 92523 reports that same speech sound evaluation combined with an evaluation of language comprehension and expression, so it is the code for a full speech and language evaluation. ASHA states that 92522 is already included as part of 92523, which is why you may only bill one or the other for the same evaluation.
ASHA's Medicare fee schedule analysis carries the same note on both codes: do not bill 92522 in conjunction with 92523. ASHA's table of Medicare correct coding edits lists the 92523 and 92522 pair as one that cannot be bypassed with a modifier, but edits are updated every quarter, so confirm the current NCCI table before billing.
When is modifier 52 used with 92523?
ASHA explains that a language-only evaluation can be reported as 92523 with modifier 52, which marks a service that was reduced compared with the full description. ASHA's Medicare coding rules say the same: for an evaluation of language only, apply modifier 52. If the clinician documents clinical judgment about speech, even if speech was assessed informally, ASHA says billing 92523 without modifier 52 is appropriate.
ASHA also directs auditory processing evaluations to 92523 with modifier 52, to show that a speech sound production evaluation was not conducted. Auditory processing treatment falls under 92507.
Are the evaluation codes timed?
The evaluation codes are not 15-minute timed codes. None of the descriptors ASHA lists for 92521 to 92524 includes a time period, ASHA identifies 92524 as untimed, and ASHA notes that most speech-language pathology codes are untimed, service-based codes. CMS says an untimed code, with 92521 as its example, is reported with one unit regardless of the minutes spent.
The Medicare 8-minute rule therefore does not apply to these codes. It only counts minutes for 15-minute timed codes. See the 8-minute rule calculator for how timed codes are counted.
Can more than one evaluation code be billed on the same day?
ASHA notes that when the codes were introduced, neither the CPT Handbook nor the Medicare correct coding edits restricted billing the new codes together in general, since a client may be evaluated for several disorders on one day. The exception is 92522 with 92523. ASHA also notes that the time for identifying other disorders is already built into the value of each code.
That guidance dates from the 2014 introduction, so check the current NCCI tables and each payer's policy for the pairs you plan to bill. Some combinations with other codes require modifier 59, and ASHA points to its correct coding edit tables for the details.
What does 92524 not include?
92524 is a behavior-based assessment of voice quality and resonance. ASHA states that it does not include instrumental assessment, which has separate codes, such as 92520 for laryngeal function studies. ASHA also notes that speaking valve and tracheoesophageal prosthesis changes are not coded with 92524.
Where does treatment code 92526 fit in?
92526 is treatment of swallowing problems or oral function for feeding. It is not part of the 92521 to 92524 family and does not evaluate speech or language. When swallowing is evaluated, the code is 92610, and 92526 reports the treatment that follows.
Evaluations and treatment are documented separately. For speech and language treatment, 92507 remains valid through December 31, 2026. See the 92507 guide for the 2027 changes.
What does Medicare require on SLP evaluation claims?
ASHA states that Medicare requires modifier GN on every code rendered under a speech-language pathology or dysphagia plan of treatment. CMS article A52866 says that CPT does not define a speech-language pathology re-evaluation code, so a re-evaluation is reported with the evaluation code, and it describes re-evaluation as not a routine, recurring service.
What should the session note show?
- What was evaluated: fluency, speech sound production, language comprehension and expression, voice and resonance. The code follows from this, so name each area.
- For 92523, evidence of both speech sound production and language evaluation. For a language-only evaluation, say that speech was not evaluated so modifier 52 is supported.
- Standardized and informal measures used and what they showed, in objective terms rather than vague severity words.
- The reason for the evaluation, relevant history and the clinical impression, with goals and a plan of care that support the services that follow.
- The total treatment time for the visit. The evaluation codes are untimed, but if timed codes are also billed, total timed minutes and total treatment time belong in the record. See the SOAP note template for a consistent layout.
Common mistakes
- Billing 92522 and 92523 on the same day for one evaluation.
- Billing 92523 for a language-only evaluation without modifier 52.
- Treating the evaluation codes as timed and reporting multiple units for a long evaluation.
- Reporting 92524 for an instrumental voice assessment, which has separate codes.
- Using 92526 for a speech or language visit, or for an evaluation of swallowing.
Frequently Asked Questions
What is CPT code 92523?
It is the evaluation of speech sound production together with evaluation of language comprehension and expression. ASHA says the speech sound evaluation (92522) is already included in it.
Can I bill 92522 and 92523 together?
No. ASHA says you may only bill one or the other, because 92522 is already included in 92523.
What does modifier 52 mean on 92523?
It marks a reduced service. ASHA uses 92523 with modifier 52 for a language-only evaluation and for an auditory processing evaluation, where no speech sound production evaluation was conducted.
What is CPT 92521?
It is the evaluation of speech fluency, how smoothly speech flows. CMS uses 92521 as its example of an untimed code that is reported with one unit regardless of the minutes spent.
What does CPT 92524 cover?
A behavior-based assessment of voice quality and resonance. ASHA says it does not include instrumental assessment, which is reported with separate codes.
Are the SLP evaluation codes timed?
No. ASHA identifies 92524 as untimed and notes that most SLP codes are untimed service-based codes, and CMS reports untimed codes with one unit regardless of minutes. The 8-minute rule does not apply to them.
How does 92526 relate to the evaluation codes?
92526 is a swallowing and feeding treatment code, not an evaluation. The swallowing evaluation code is 92610. 92526 is outside the 92521 to 92524 family.
