How is the PHQ-9 scored?
The PHQ-9 is a nine-item depression questionnaire. Each item is scored 0 to 3 and the items are added for a total of 0 to 27. The developers' cut points of 5, 10, 15 and 20 mark mild, moderate, moderately severe and severe depression. Record the total, the band and your clinical interpretation.

The Patient Health Questionnaire-9 (PHQ-9) is the depression module of the PHQ, developed by Drs. Robert L. Spitzer, Janet B.W. Williams, Kurt Kroenke and colleagues. It asks how often the client has been bothered by nine depression symptom domains over the last 2 weeks, so each administration describes a two-week window.
Therapists use it at intake and at set intervals to show change over time. This guide has a free scorer that runs in your browser, a lookup for every total from 0 to 27, a helper for comparing two administrations, the item 9 safety question, where the result belongs in a SOAP note or DAP note, and the usage terms. It does not reproduce the questions: use the official form linked in the sources.
Score a PHQ-9 now
Enter the client's answer for each of the nine items from a completed PHQ-9 form. The labels are short topic names, not the item wording: use the official form to administer it. The final difficulty question on the form is not added to the total, so it is not part of this scorer. This runs in your browser: nothing you enter is sent or saved.
0 of 9 items answered. Answer all nine to see the total and severity band; a partial sum is not a PHQ-9 score.

PHQ-9 severity bands
| Total score | Severity band | Source note |
|---|---|---|
| 0 to 4 | Minimal | Lowest band in Table 2 of the validation paper. |
| 5 to 9 | Mild | A score of 5 is the lower limit of mild. |

| 20 to 27 | Severe | A score of 20 is the lower limit of severe. |
What does each PHQ-9 total mean?
Every total from 0 to 27 falls in exactly one band. The table maps each total to its band and says where it sits, which answers questions like "what does a PHQ-9 of 9 mean" (the top of mild, one point below the moderate cut point) or "what does 21 mean" (severe).
| Total | Band | Where it sits |
|---|---|---|
| 0 | Minimal | Lowest possible total: every item answered "not at all". |
| 1 | Minimal | Within the minimal band (0 to 4). |
| 2 | Minimal | Within the minimal band (0 to 4). |
| 3 | Minimal | Within the minimal band (0 to 4). |

What do the PHQ-9 cut points mean?
The validation paper states that "PHQ-9 scores of 5, 10, 15, and 20 represent valid and easy-to-remember thresholds demarcating the lower limits of mild, moderate, moderately severe, and severe depression." The bands in the table above follow from those four cut points and the 0 to 27 range.
The same paper reports that, against a mental health professional interview, a PHQ-9 score of 10 or higher had a sensitivity of 88% and a specificity of 88% for major depression. That is a statement about a screening threshold in a primary care study. It is not a diagnosis, and a score alone never replaces a clinical interview.
How should you handle PHQ-9 item 9?
Item 9 is the item on thoughts of death or self-harm. Its score is added into the total like every other item, so a client can land in a low severity band and still have answered item 9 above zero. Because the band does not tell you that, document the item 9 response on its own line every time. The scorer above flags any item 9 answer above zero for the same reason.
What you do next is a clinical decision under your setting's protocol. The PHQ instructions note that a four-item screener has been developed to help with positive responses to item 9, and that a final decision about the actual risk of self-harm requires a clinical interview. For related references, see the C-SSRS documentation guide, the risk assessment template and the safety plan template. This page does not set out a triage procedure.
How do you show change across sessions?
Record the total and the band each time the PHQ-9 is given, with the date, and state the prior score next to it. A line such as "14, down from 18 at intake" lets a reviewer see direction without opening earlier notes. Keep the interpretation in your own words in the Assessment section: the form gives you a score and a band, but connecting it to the client's presentation and the treatment plan goals is your clinical judgment.
How big a change matters? In a study drawn from 434 older adults (mean age 71) in a late-life depression treatment trial, Lowe and colleagues estimated the minimal clinically important difference for individual change at 5 points on the 0 to 27 scale, using subgroups who completed the PHQ-9 twice, 7 days apart. That is one study population, so if your agency, payer or supervisor sets a different threshold, use theirs, and write any target into the depression treatment plan. The helper below shows the point change, the band change, and whether the change reaches 5 points.

How is the PHQ-9 different from the PHQ-2, PHQ-8 and PHQ-A?
They belong to the same PHQ family, and the PHQ instructions describe each one. The PHQ-2 is the first two items of the PHQ-9 and is used as an ultra-brief screener; a score of 3 or more should lead to the full PHQ-9 and a clinical interview. In its validation study, a PHQ-2 score of 3 or more had a sensitivity of 83% and a specificity of 92% for major depression. The PHQ-8 drops item 9, so it does not ask about thoughts of death or self-harm.
The PHQ-A is a substantially modified version of the PHQ developed for adolescents, and the instructions say its validity data are moderate but much less than for the original PHQ. Separately, a study of the PHQ-9 itself in 442 youth aged 13 to 17 found a cut point of 11 or more worked best for detecting major depression (sensitivity 89.5%, specificity 77.5%). Use the version and cut point your setting has adopted, and name the version in the note.
| Measure | Items | Score range | What the sources say about use |
|---|---|---|---|
| PHQ-9 | 9 | 0 to 27 | Depression severity, with cut points of 5, 10, 15 and 20. |
| PHQ-2 | First 2 items of the PHQ-9 | 0 to 6 | Ultra-brief screener. A score of 3 or more should prompt the full PHQ-9 and a clinical interview. |
| PHQ-8 | All PHQ-9 items except item 9 | 0 to 24 | Scored like the PHQ-9 with the same cut points. Mainly used in research studies not focused on depression. |
| PHQ-A | Modified PHQ for adolescents | Not stated in the instructions | Moderate validity data, much less than for the original PHQ. Diagnostic scoring is in a manual available on request. |
How do you document it in a progress note?
- Step 1: State that the PHQ-9 was given, the date, and who completed it (client self-report, or read aloud).
- Step 2: Record the total score (0 to 27) and the severity band.
- Step 3: Record the item 9 response separately, and what you did about it if it was above zero.
- Step 4: Compare with the previous score and name the trend: improved, unchanged or worsened.
- Step 5: Put your interpretation in the Assessment: how the score fits the client's presentation, history and goals.
- Step 6: Tie the plan to the result: continue, adjust interventions, or repeat the measure at a stated interval.
Where the PHQ-9 goes in each note format
| Note format | Where the score goes | Where the interpretation goes |
|---|---|---|
| SOAP | Objective: total, band, item 9 result, prior score. | Assessment: what the score and trend mean for this client. |
| DAP | Data: total, band, item 9 result, prior score. | Assessment: what the score and trend mean for this client. |
Example wording (fictional, for illustration only)
Objective: PHQ-9 completed in session on 10/09, total 14 (moderate band), down from 18 at intake. Item 9 scored 0. Assessment: Depressive symptoms have eased but remain in the moderate range; client reports more interest in activities since starting behavioral activation. Plan: continue weekly sessions and repeat the PHQ-9 at the next treatment plan review.
Can you use and reproduce the PHQ-9 for free?
The PHQ-9 form on phqscreeners.com carries the notice "Developed by Drs. Robert L. Spitzer, Janet B.W. Williams, Kurt Kroenke and colleagues, with an educational grant from Pfizer Inc. No permission required to reproduce, translate, display or distribute." The site also states that all PHQ and GAD-7 screeners and translations are downloadable there and that no permission is required to reproduce, translate, display or distribute them, and the PHQ instructions say all the measures in the PHQ family, including the PHQ-2, PHQ-8 and PHQ-A, are in the public domain.
The site's terms of use add that Pfizer makes no warranties about the content and that use is at your own risk. WellNotes links to the official form instead of reproducing the questions, and the scorer on this page uses short topic labels, not the item wording. Nothing on this page is a substitute for the form or its terms.
Frequently Asked Questions
What is a normal PHQ-9 score?
The PHQ-9 does not define a normal score. A total of 0 to 4 falls in the minimal band and 5 to 9 in the mild band, and lower scores mean fewer reported symptoms. What counts as a good score for a given client depends on their starting point and treatment goals, so compare against their own earlier scores.
Is a PHQ-9 score of 10 positive?
A score of 10 or higher is the screening threshold studied in the validation paper, where it had a sensitivity of 88% and a specificity of 88% for major depression against a clinician interview. The PHQ instructions call 10 or more a yellow flag for a possible clinically significant condition. It is not a diagnosis, which requires a clinical evaluation by the treating clinician.
Is item 9 part of the PHQ-9 total?
Yes. Item 9 is scored 0 to 3 and added to the total. Because a low total can still include a positive item 9, document the item 9 response separately and the follow-up you completed.
What does the last question about difficulty mean?
The form ends with a question on how difficult the problems have made work, home life or getting along with other people. The PHQ instructions say it is not used in calculating any PHQ score and that it reflects the client's overall impression of symptom-related impairment. If you use it, record it separately from the total.
Where does the PHQ-9 go in a SOAP note?
Put the total score, severity band, item 9 result and prior score in the Objective section. Put your interpretation of what the score and trend mean for the client in the Assessment section.
Do I need permission to use the PHQ-9 in my practice?
The official form on phqscreeners.com states that no permission is required to reproduce, translate, display or distribute it, and the PHQ instructions say the PHQ measures are in the public domain. Read the site's terms of use and confirm with your agency if it has its own approved forms.
Can I score the PHQ-9 online for free?
Yes. The scorer on this page adds up the nine item scores you enter and shows the total, the severity band and an item 9 flag. It runs in your browser, and nothing you enter is sent or saved. You still interpret the result.
Does WellNotes score the PHQ-9 for me?
No. The WellNotes note app does not administer or score the PHQ-9; it drafts your session note from what you type. The free scorer on this page only adds up the item scores you enter, in your browser, and nothing is saved. You interpret the score, write it into the note and review the note before it is used.
