How should bipolar disorder show up in each session note?

The F31 code changes with the clinical picture, so the note has to keep up. Each note should say what the current or most recent episode is, how severe it is, and whether psychotic features are present, or that the client is in partial or full remission. When the clinician documents a shift, such as from a depressed episode to remission, the code on the next claim changes too.

Example assessment line for a SOAP note: "Bipolar II disorder (F31.81). Mood stable over two weeks; sleep 7 to 8 hours; no hypomanic symptoms reported.
