ICD-10 Codes for Schizophrenia (F20) and Related Psychotic Disorders

Codes verified against the CMS FY2027 ICD-10-CM code descriptions (effective October 1, 2026; these codes are unchanged from FY2026). Reviewed . See sources.

What is the ICD-10 code for schizophrenia?

Schizophrenia is coded in ICD-10-CM category F20. F20.9, Schizophrenia, unspecified, is where the index sends schizophrenia with no type stated. F20.0 to F20.5 cover the paranoid, disorganized, catatonic, undifferentiated and residual types, F20.81 is schizophreniform disorder and F20.89 is other schizophrenia. F20 and F20.8 are headings, not billable codes.

Category F20 opens the FY2027 ICD-10-CM block "Schizophrenia, schizotypal, delusional, and other non-mood psychotic disorders (F20-F29)" in Chapter 5. Its subtype codes come from older diagnostic systems. The American Psychiatric Association removed the schizophrenia subtypes from DSM-5, but ICD-10-CM still lists them, so the code on the claim follows the diagnosis the treating clinician documents.

This page lists every F20 code with its official FY2027 description and inclusion terms, the schizoaffective and other psychotic disorder codes in F21 to F29, the Excludes notes that decide between them, and what a session note should show. For the procedure side of the claim, see the CPT code reference.

Which F20 code fits the diagnosis?

Start with the words in the record. In the FY2027 Alphabetic Index, the main term "Schizophrenia, schizophrenic" points to F20.9, so a diagnosis of schizophrenia with no type stated is coded F20.9. The subterms send paranoid schizophrenia to F20.0, disorganized or hebephrenic to F20.1, catatonic to F20.2, atypical or undifferentiated to F20.3, residual to F20.5, and simple, cenesthopathic or "specified type NEC" to F20.89. There is no F20.4: the codes run from F20.3 to F20.5.

The Official Guidelines base code assignment on the provider's diagnostic statement, not on the clinical criteria the provider used (Section I.A.19), and ask for the highest level of specificity the record supports (Section IV.F.3). A subtype code is right only when the clinician documents that subtype. Category F20 also carries a "use additional code" note: when it applies, add a code from R41.84- to identify another specified cognitive deficit.

Does DSM-5-TR still use schizophrenia subtypes?

No. The APA's DSM-5 fact sheet on schizophrenia says the diagnostic criteria no longer identify subtypes, because clients' symptoms often moved from one subtype to another and overlapped, which blurred the distinctions and lowered their validity. Catatonia became a specifier that can be used with schizophrenia and with other conditions such as schizoaffective disorder, bipolar disorders and major depressive disorder. DSM-5-TR, published in 2022, is the text revision of DSM-5.

ICD-10-CM did not follow that change, so the subtype codes F20.0 to F20.5 remain valid. In practice, a record that follows DSM wording and says only "schizophrenia" is coded F20.9. Several DSM diagnosis names appear in the FY2027 tabular list as inclusion terms or in the index, which makes the mapping below traceable to the official files.

F20.9Schizophrenia, unspecifiedYesSchizophrenia with no type stated. The index also sends "chronic undifferentiated" schizophrenia here.
F20.81Schizophreniform disorderYesSchizophreniform disorder. The index entry Psychosis, schizophreniform also points here.
F25.0Schizoaffective disorder, bipolar typeYesSchizoaffective disorder, bipolar type. Inclusion terms also cover manic and mixed type.
F25.1Schizoaffective disorder, depressive typeYesSchizoaffective disorder, depressive type.
F23Brief psychotic disorderYesBrief psychotic disorder. The index sends acute (brief) schizophrenia here too.
F22Delusional disordersYesDelusional disorder.
F21Schizotypal disorderYesInclusion term: schizotypal personality disorder.
F28Other psychotic disorder not due to a substance or known physiological conditionYesInclusion term: other specified schizophrenia spectrum and other psychotic disorder.
F29Unspecified psychosis not due to a substance or known physiological conditionYesInclusion terms: unspecified schizophrenia spectrum and other psychotic disorder, psychosis NOS.

What is the ICD-10 code for schizoaffective disorder?

Schizoaffective disorder has its own category, F25, with four billable codes. Category F25 has an Excludes1 note for schizophrenia (F20.-) and for mood disorders with psychotic symptoms, and F20 has a matching Excludes1 note for schizoaffective disorder (F25.-) and cyclic schizophrenia (F25.0). Excludes1 means the two codes are not reported together for the same condition (Section I.A.12.a), so a record that documents schizoaffective disorder uses an F25 code instead of an F20 code.

F25 schizoaffective disorder codes
CodeOfficial description (FY2027)BillableWhen it fits
F25Schizoaffective disordersNo (header)Category only.
F25.0Schizoaffective disorder, bipolar typeYesInclusion terms: cyclic schizophrenia, schizoaffective disorder manic type or mixed type, schizoaffective psychosis bipolar type.
F25.1Schizoaffective disorder, depressive typeYesInclusion term: schizoaffective psychosis, depressive type.
F25.8Other schizoaffective disordersYesOther specified schizoaffective disorders.
F25.9Schizoaffective disorder, unspecifiedYesInclusion term: schizoaffective psychosis NOS.

Which codes cover related psychotic disorders?

The rest of the F20 to F29 block covers psychotic disorders that are not schizophrenia. F20 has an Excludes1 note for brief psychotic disorder and schizophrenic reaction NOS (both F23), and for mood disorders with psychotic symptoms (F30.2, F31.2, F31.5, F31.64, F32.3, F33.3). Those are coded in the bipolar and depression categories.

F20 also has an Excludes2 note for schizophrenic reaction in alcoholism (F10.15-, F10.25-, F10.95-), psychoactive drug use, brain disease or epilepsy (F06.2), and for schizotypal disorder (F21). Psychosis induced by a substance is coded in the substance category, as the substance use code guide explains.

F06.2Psychotic disorder with delusions due to known physiological conditionYesPsychosis the provider attributes to a medical condition, such as brain disease or epilepsy.

Is there an ICD-10 code for chronic or simple schizophrenia?

Simple schizophrenia is coded F20.89, Other schizophrenia. "Simple schizophrenia" is an inclusion term under F20.89, and the index lists simple (type) and simplex under Schizophrenia with the same code.

There is no separate code for chronic schizophrenia. The index has no stand-alone "chronic" subterm under Schizophrenia; it sends "chronic undifferentiated" schizophrenia to F20.9, both as Schizophrenia, chronic undifferentiated and as Schizophrenia, undifferentiated, chronic. Undifferentiated schizophrenia without the word chronic is F20.3. When the record says chronic schizophrenia and nothing more, the main term applies and the code is F20.9.

Which CPT codes are billed with schizophrenia diagnoses?

The ICD-10-CM code says why the client is being seen; the CPT code says what service was provided. Therapists bill individual psychotherapy by time, such as CPT 90834 or CPT 90837, and the initial evaluation as CPT 90791. Prescribers often bill an E/M visit such as 99214, adding 90833 when they also provide psychotherapy. Which diagnoses and services a plan covers varies by payer. Confirm with the payer before billing.

What should the session note show?

  • The diagnosis in words as the treating clinician states it, including the subtype or schizoaffective type when one is documented, so it matches the code on the claim.
  • Symptoms reported and observed this session, such as hallucinations, delusions, disorganized speech or behavior, and negative symptoms, in observable terms.
  • Functional status: self-care, housing, work or school, and relationships, and any change since the last visit.
  • Risk assessment when indicated, and safety planning steps when taken. The risk assessment template lays out the parts.
  • Medication adherence as the client reports it, and coordination with the prescriber when it happened, with who was contacted and when.
  • The intervention used, the client's response, and progress toward a treatment plan goal.
  • Draft the session note in SOAP or DAP format so the assessment and plan are easy to find. The therapy progress note generator drafts the note from what you type, and you review it.

Common mistakes

  • Reporting F20 or F20.8 without the remaining characters. They are headings, so the claim is invalid.
  • Choosing a subtype code such as F20.0 when the record documents only "schizophrenia". Use F20.9.
  • Reporting an F20 code together with an F25 code for the same condition despite the Excludes1 notes.
  • Coding a mood disorder with psychotic features, such as major depressive disorder with psychotic symptoms, as schizophrenia.
  • Looking for F20.4. It does not exist in ICD-10-CM.
  • A diagnosis on the claim that never appears in the session note.
