ICD-10 Code for Suicidal Ideation: R45.851

Codes verified against the CMS FY2027 ICD-10-CM code descriptions (effective October 1, 2026; these codes are unchanged from FY2026). Reviewed . See sources.

What is the ICD-10 code for suicidal ideation?

The ICD-10-CM code for suicidal ideation is R45.851, Suicidal ideations. It is billable and sits in subcategory R45.85 next to R45.850, Homicidal ideations. R45.85 has an Excludes1 note for suicide attempt (T14.91), so the two are not reported together. A personal history of suicide attempt is Z91.51.

If a client is in immediate danger, call 911 or local emergency services. For crisis support, call, text or chat 988 (Suicide and Crisis Lifeline). SAMHSA lists a suicide attempt in progress as an example of when calling 911 is appropriate, and describes 988 as 24/7 support. This page is a coding reference. It does not assess risk, and a diagnosis code is not a risk assessment.

R45.851 is a symptom code in Chapter 18 of ICD-10-CM, not a diagnosis. This page lists the related codes with their official FY2027 descriptions, explains the Excludes1 note that separates ideation from attempt, and shows what a session note should contain so the code is supported. For the clinical documents themselves, see the safety plan template and the risk assessment template.

Can R45.851 and T14.91 be reported together?

Not for the same condition. R45.85 carries an Excludes1 note for suicide attempt (T14.91). The Official Guidelines say an Excludes1 note means the excluded code should never be used at the same time as the code above the note, with one exception: when the two conditions are unrelated to each other. If it is not clear whether the conditions are related, the guidelines say to query the provider (Section I.A.12.a).

The same logic separates the active record from the past. A client who is currently having suicidal ideation has R45.851 documented. A client who attempted suicide at some earlier time and has no current treatment for it can have a personal history code, which the Guidelines describe as a past condition that no longer exists and is not receiving any treatment, but that has the potential for recurrence and may require continued monitoring (Section I.C.21.c.4). Which applies is the clinician's documented finding.

Is suicidal ideation coded separately when the client has depression?

The Official Guidelines give two general rules. Signs and symptoms associated routinely with a disease process should not be assigned as additional codes, unless the classification instructs otherwise (Section I.B.5). Additional signs and symptoms that may not be associated routinely with a disease process should be coded when present (Section I.B.6). The tabular list adds an Excludes2 note at the head of R40 to R46 for symptoms and signs constituting part of a pattern of mental disorder (F01 to F99).

The FY2027 guidelines do not address suicidal ideation by name, so this is a coding judgment to make from the clinician's documentation and the payer's guidance. The code only records that ideation was documented. It carries no information about severity, intent, plan or timing, so those belong in the note.

Can a symptom code be the first-listed diagnosis?

The Guidelines say codes for symptoms and signs are acceptable when a related definitive diagnosis has not been established by the provider (Section I.B.4), and that in outpatient care the first-listed diagnosis may be a symptom when a diagnosis has not been established (Section IV.G). When a diagnosis such as a mood disorder is established and documented, that diagnosis is the one to build the claim around. Confirm payer rules before billing.

How does the ideation code connect to the CPT code?

The ICD-10-CM code says why the client is being seen; the CPT code says what service was provided. A session that addresses suicidal ideation is reported with the psychotherapy code that matches the time and service, such as CPT 90837, and a crisis session has its own codes. See CPT 90839 for psychotherapy for crisis. The diagnosis must be supported in the note for every date of service.

What should the session note show?

  • What the client said, in the client's own words where possible, and the date, so the ideation is documented for this session.
  • The clinician's assessment of that ideation, in the clinician's words. The code does not carry severity, intent, plan or timing.
  • Any structured screen or risk assessment used, with the instrument named, the date and the result the clinician recorded.
  • Protective factors and supports the clinician identified, and any safety planning done in the session.
  • Actions taken and follow-up arranged, with who was contacted and when, if anyone was.
  • Any history of attempt or self-harm, kept separate from current ideation so the correct code type is supported.
  • Draft the session note in SOAP or BIRP format. WellNotes drafts the note from what you enter; you review and edit it.

Common mistakes

  • Reporting R45.85 without the sixth character. R45.85 is a subcategory heading.
  • Reporting R45.851 and T14.91 together for the same episode despite the Excludes1 note.
  • Using T14.91 without a seventh character.
  • Using a history code (Z91.51) for ideation that is current.
  • Coding R45.850 when the record documents suicidal ideation, or the reverse.
  • A note that records the code but not what the client said or what the clinician assessed.
